Medical Plan Participants: Wellness Pricing

Employees participating in Colas/Simon Medical Insurance Plans, please note:

 To continue eligibility for “wellness plan pricing” on premiums next year (2025), employees (and spouse, if applicable) must complete a “well check” exam with their primary care physician this year by 10/31/2024. 

• It is important to ask that the visit be coded as a “routine preventive/wellness check” by your doctor
• The well-check visit must be with a primary care physician

Well-check appointments are covered at 100% by the medical plans, however, failure to complete the required annual well-check will result in higher monthly premiums in 2025.
