Simon has partnered with ADP to provide employees the option to view pay statements online or receive paper statements in the mail. Please choose ONE option below regarding your choice for receipt of your pay statements/paystubs*. Your election can be changed in the future by completing and submitting this form.
*Electronic access is strongly encouraged - this free service allows you to view your paystubs online, anytime at www.myadp.com.

I elect to receive paper copies of my pay statements. I understand, by electing this option, my pay statements will be mailed via USPS. Pay statements will be mailed to my current mailing address on file, and Simon cannot be held responsible for delayed receipt of pay statements due to weather or other factors outside of our control affecting postal service processing.
I elect to access my pay statements online via the ADP website at www.myadp.com, and/or via the ADP mobile app (available for free download on Android or iOS devices). I understand by electing this option, I will not receive paper statements in the mail. I also understand that access to my electronic pay statements requires that I create a separate online user account through ADP, and maintain my own username and password to access the website and/or mobile app.
